Where is the surge in COVID-19 deaths?

We have yet to see a surge in COVID-19 deaths to follow from the surge in confirmed cases. Some attribute this to the disease moving to a younger demographic, or to better medical care.
In a country as large and diverse as the US, aggregating numbers in a time series, particularly when there is a 2-3 week delay between diagnosis and death, can give a misleading picture. This link discusses that possibility.
"The lag time for new increases in deaths, plus the lag time for the low incidence of deaths in the Northeast where cases peaked over 2 months ago, suggests that more is going on than just younger demographics and better medical treatments. The delay in deaths works both ways."
